Perkins Loan Request Form

Perkins Loan Request Form is no longer available.

The Perkins Loan Program has expired as of September 30, 2017. If you were awarded a Perkins Loan for 2017 - 2018, and the Fall 2017 portion disbursed prior to October 1 you will still receive your Spring 2018 disbursement.

We do not anticipate making any new Perkins Loans for the 2018 - 2019, 2019-2020 or 2020-2021 academic years.

Note: Documents may be faxed, mailed, or brought in during office hours. Contact a financial aid counselor for help completing any form. The student's name and ISU ID number should be clearly written on every document submitted, including student and parent tax returns.
